Michelle Leonhardt

Executive Director, Quality & Value-Based Performance

Michelle Leonhardt, RN, MBA, is an experienced clinical operations leader with over 20 years of progressive leadership across value-based care, population health, and senior-focused care delivery models. She has led multi-state interdisciplinary teams—including RNs, Community Health Workers, social workers, pharmacists, and care coordinators—driving improvements in transitions of care, high-risk patient management, patient engagement, and quality outcomes. Michelle has deep experience in post-acute and long-term care management, with a strong record of establishing processes that improve quality outcomes, ensure patients receive the right care at the right time, and strengthening collaboration across care settings. Her work has consistently contributed to improved patient outcomes, utilization patterns, and reductions in total cost of care for complex and high-risk populations. She brings a strong blend of clinical expertise and operational strategy. In previous roles, she built and optimized care management programs that improved readmission rates, strengthened PCP follow-up, enhanced patient experience, and generated measurable reductions in avoidable utilization. She has extensive experience partnering with physicians, market leaders, community providers, and health plan partners to implement evidence-based workflows, develop team competencies, and design scalable processes that align with organizational goals. Her passion for people-centered care and continuous improvement aligns closely with Spectrum Medical Partners’ mission to deliver high-quality, integrated care across post-acute and population health settings. Michelle looks forward to supporting SMP’s outcomes by bringing strong collaboration, operational discipline, and a commitment to meaningful patient and organizational results. Outside of work, Michelle enjoys traveling, spending dedicated time with her family, exploring new cities, and staying active. She has one daughter, age 24, who is majoring in child psychology—something Michelle is incredibly proud of. She brings warmth, curiosity, and a team-oriented spirit to every environment she joins.
